We offer on-line claim reporting as a convenience to our clients. You may complete the information requested on the appropriate form or call us Monday thru Friday between 8:00am and 5:00pm EST. You may also contact your insurance company directly, either by phone or on-line. Click here to review the list of insurance companies. If you do report a claim directly to the insurance company, please let us know so we may track the claim for you. If you choose to report your claim on-line and do not receive a confirmation by 11:00am the next business day, please call us to confirm receipt.
